Services
The nurse navigators help you and your child or young person:
- understand your healthcare needs and the healthcare system
- navigate the health care system to access services that help promote better health outcomes and care closer to home.
They also:
- work with community and local care providers including GPs, hospitals, Aboriginal & Torres Strait Islander Medical Services, other community health services and non-government organisations to support your child’s or young person's care
- provide evidence-based care to promote better patient outcomes and, where possible, optimize each hospital visit and provide family centred care
- provide an integrated health care summary when indicated to help inform others of your child's health care needs
- support you when your child or young person returns home and make sure you have a local primary care provider
- listen to your feedback to help improve systems and processes in your child’s or young person's healthcare journey.
If you live in a regional, rural or remote area, the nurse navigators coordinate your care through other nurse navigators in your area.
The Nurse Navigators work with Aboriginal and Torres Strait Islander support services, the connected care program and other nurse navigators in adult services.
Who can use this service?
Children and young people aged 16 and under with complex healthcare needs who live in Queensland and northern New South Wales can use this service.
Young people aged 17 and 18 can use this service if they’re already under the care of our hospital.
Who can use this service?

Young people aged 17 and 18 can use this service if they’re already under the care of QCH.
How to use this service
You need a referral from one of the following to use this service:
- paediatric specialists
- hospital-based medical officers
- nurses
- allied health professionals.
