Our role

We review clinical incident reports involving children and young people across Queensland’s public hospitals and health facilities.

A clinical incident is an unexpected event during health care which causes or may cause unintended harm to a patient. We review Queensland incidents with a severity assessment code 1 (SAC 1). These are incidents involving likely permanent harm or death.

When a SAC 1 clinical incident happens, the hospital or health service does a review to identify what caused it. They also make recommendations to stop it happening again. The reviews include formal clinical incident reports and root cause analysis. We review these reports and coronial findings using a multi-incident analysis method to help:

  • identify common themes and contributing factors
  • reduce the risk of clinical incidents
  • make recommendations to improve the safety and quality of healthcare across Queensland.

  • patient safety
  • human and systems factors
  • surgery
  • retrieval services
  • rural and remote services.

We have monthly meetings to review clinical incidents and identify opportunities for improving the quality and safety of health care.
