Therapeutic drug monitoring (TDM) advice
You can obtain advice on Therapeutic Drug Monitoring (TDM) from your ward pharmacist (via pager) or by contacting the Queensland Children’s Hospital (QCH) Pharmacy Department.
Paediatric TDM differs from that relating to adults, so please don’t hesitate to contact your Clinical pharmacist for advice regarding starting doses, or advice on interpreting blood levels and advice regarding appropriate dose adjustments.
Aminoglycosides
Gentamicin and Tobramycin

Note: Interpretation of the plasma drug levels require knowledge of patient’s condition, weight, height, serum creatinine, drug dose, exact time of commencement and end of infusion (including mode of administration) and exact times of blood collections.
