Accessing patient health information

The Health Information Access team at the Queensland Children’s Hospital (QCH) provides advice and guidance to clinicians and the wider community on all aspects of accessing health information and is the first point of contact for enquiries about privacy and confidentiality.

Types of information accessible include:

  • discharge summaries
  • pathology results
  • correspondence
  • medical imaging reports
  • inpatient notes
  • outpatient notes
  • operation reports
  • anaesthetic records
  • diagnostic reports

Requests in writing are preferred to ensure the information is released in compliance with the Hospital and Health Boards Act 2011 using your practice letterhead. In the case of information required in a medical emergency, please phone the numbers above.

Medical emergencies will receive priority over routine requests. Most requests are processed within 48 hours. Please request ahead for appointments where possible.
