Our commitment statement

We're committed to being open and transparent and allow access to information held by our health service. We can give you most kinds of information unless there's a good reason why we can't.

What is right to information

The Right to Information (RTI) process is the Queensland Government's approach to giving the community greater access to information.

The Queensland Government has made a commitment to allow the public to access information they have unless it's not in the best interests of the public.

There are two main laws that govern this:

Information about you

You have the right to access your personal information that we hold.

To find out how to access your health records, call Health Information Access on 07 3068 5372 or email CHQ_HIA@health.qld.gov.au.

Parents and carers can request the information that we have on behalf of their child.

Find out how to request access to your child’s health records.

If we have the wrong information, you have the right to ask us to change it.

Information about us

You can get information about us and how we work.

There's a formal process you can use to apply for information, but you should try informal ways first. It can save you time and money.

1: Check online

The information you're looking for might already be online. Check these places first:

2: Check with us

You can also contact us to ask for information. Our staff may be able to help you find the information you need without having to go through a formal process.

3: Make a formal RTI request

If you can’t find the information online or get it from us informally, you can make a formal request on the Queensland Government website.

If you’d like information about Queensland Health, select Apply online, then select Queensland Health in the Department section.

If you’d like information about us, download and complete the RTI PDF form and submit it.

What happens next

Sometimes we're not able to release information. For example, we can't release something if it would be against the public interest.

If you’re not satisfied with the decision, you can apply to have it reviewed. You need to do this in 20 business days from the date on the letter telling you about our decision.

You can also ask the Information Commissioner to review our decision.
